Can anyone recommend a free site that does AI image-to-image generation, and handles NSFW images?
I've found a few through Google searches that say they do, but then reject the image as being against their rules.

I use several AI application sites online.

I can give you a link to one... Not exactly what you are asking for, but it works for me...
You add a picture and then click on 'Caption' ; it then gives you a written prompt in about 10 seconds.

for generating prompts when I can't think of the right wording myself.
But it's the image-to-image stuff that I'm really looking for - when you have a good image, but it just needs a few minor tweaks!
